Before you visit, download and fill out the new patient information forms. See new patient forms under the menu tab. This will speed up the office process for you. Please bring all of your insurance information as well as a valid photo ID and your copay. Bring all imaging studies reports that have been performed including x-rays, MRIs and CT scans. We do not need the disc or the films. Dr. Mathews will review the reports with you and explain any important findings and then discuss treatment options for you.

We can usually get new patients scheduled within one to two weeks as long as we have received all demographic, insurance, and medical records from your physician’s office.

Yes, in most instances you will need a referral from your doctor. We do need your demographic, insurance, and medical records before the referral can be reviewed and an appointment can be scheduled.

We do not call in prescription refills. You have to make an appointment and be seen by the doctor or the physician assistant to make changes on your prescriptions or request refills.

Yes, all co-pays and co-insurances are due at time of service.

Yes, we do accept self-pay patients.

We accept most commercial insurance companies including Medicare, Blue Cross and Blue Shield, United Health Care, Cigna, Aetna, Tricare, and Welforce. You may call to speak with our billing department to confirm if your insurance is accepted.
